A project manager learned that a corrective action was taken by a team member and was not documented. What should the project manager do FIRST?
    SELECT THE CORRECT ANSWER

A.Report the violation to the functional manager
B.Clarify the reasoning behind the team member's action
    C. Assess if the action should be documented
    D. Find out who caused the problem

    -------------------------------------------------------------------------------------------------------------------------------------------------
I chose B but correct one is C.
Justification: Since the corrective action is not documented, the next step is to document. The Project Manager needs to assess if the action should be documented.

My assessment is given the corrective action has already been implemented, the project manager do not really have a choice to "NOT" document it (and hence can't assess if it should be documented or not). So, the next best answer would be to know the reason behind the action (what was wrong at first place that led to corrective action - finding the root cause). Please let me know your views.

    First pass:

Analyze first. This gives us B, C.

What are the differences between B and C? B looks at the member's action, where C looks at the issue. Hmmmm.

    Second pass:

Scenario: Corrective action was not documented (uncontrolled change).

Real Question: What does a PM do *first*?

B: What caused the team member to take action?
C: What is the issue? What is the impact to the project?

B is not bad; we can correct behaviors and minimize future potential issues. However, dealing with the current issue - quickly and efficiently - is what we should do. C is the preferred answer.
